Film ini mengisahkan kegelisahan dan ketakutan penduduk Kota London yang terinfeksi virus mengerikan. Enam bulan virus ini mengganas dan membinasakan Inggris. Dibantu Angkatan Bersenjata Amerika Serikat Inggris berhasil melawan virus ini. Rekonstruksi negara bisa dimulai.
Kota London juga kembali aman. Sebagian penduduk yang selamat mulai kembali masuk kota. Sebagai gelombang pengungsi pertama sebuah keluarga bersatu kembali. Mereka adalah Don (Robert Carlyle), dan teman wanitanya Scarlett (Rose Byrne). Istri Don meninggal setelah terserang virus.
Dua bocah Tammy dan Andy yang bisa bertemu kembali dengan ayahnya. Namun ia tidak menduga bisa bertemu dengan ibunya, bernama Alice yang mereka kira sudah tewas. Hanya saja Alice masih terifeksi. Saat itu muncul masalah serius. Tim dokter tidak sepakat, apakah Alice harus dibunuh atau diselamatkan. Sementara Alice sendiri mulai menularkan virus itu ke suaminya. Dan, bencana itu kembali terjadi.
Film ini akan lebih menarik jika anda mengikuti prekuel sebelumnya, Film 28 Days Later yang digarap Danny Boyle pada 2002.
